Have you worked with support on this yet?  If not, the first thing to check is to make sure that your servers are in the Recommended Server Defaults policy, if it's servers that this is happening to.

 

We're actively investigating this issue so please contact support (or I can have them reach out to you) if you have systems that aren't reporting in.
