I ran into this on a friend's PC. Primarily, how does this malware get into the PC in the first place? Is it usually through an offer of "free software" that people download and install? Attached as a file on an email? Or by other means as well?

The infections vector could be any of the ones that you have mentioned. IT very much depends on the 'variety' or version of the locker functionality that has managed to get onto the system.
